Identification for the VoS Application 

The following applies whether undertaking remote or face to face casework:

In line with the MOD identification requirement you need to see proof of identity of the person who served, such as a passport or driving license. Proof of address should also be seen, such as utility bills, council tax letter, bank statement, credit card letter, etc)

Where a drivers license or passport is not available, ask for official documents which link to the eligible serving person. Such forms of ID are birth certificate, utility bills, council tax letter, bank statement, credit card statement etc.  At least two forms of ID should be provided and these should match the information on the VOS form.

This will enable you to complete the first declaration on the VoS Application form, confirming that you "Have seen documents that verify the identity of the client making this request."

If the person served under a different name

If someone served under a different name, and has since changed their name, we must receive documentation/ identification which establishes the link between the previous and current name. Without this we cannot provide financial assistance, as we cannot guarantee that the individual is actually the person who served.

If the person whose Service details are being requested is deceased a death certificate or proof of death, for example an invoice from undertaker or confirmation of Bereavement Allowance (formerly Widow’s Pension) is required.

In cases where the service/ex-service person is unable to sign, a copy of power of attorney and the attorney’s consent should be attached. Where there is no power of attorney, a note from a medical professional explaining the client’s incapacity will suffice.

In cases of estrangement or divorce, where the service/ex-service person has not signed, and as much information as possible has been provided, the Service Verification office will only be able to confirm or deny service, as under data protection laws no additional details can be released.

Any request received with insufficient documentation will be returned.

If the case is time sensitive state this and a brief reason why in the email to the disclosure cell. Mark the email as high importance and add URGENT to the subject heading of the email.
