This form is for Caseworkers to use when client verbal consent has been gained by the Regional Office (RO).

It can be used to collect the client information for Mosaic Stage 2 'Application for Assistance' - it is the a Form A without the section 2 'Declaration' as this has been performed by the RO.

It is for internal use only, and should not be sent to the client for completion.
