This form enables SSAFA volunteers and employees to obtain consent to enable SSAFA to request personal/sensitive information from an external organisation. Information that might otherwise be withheld, due to General Data Protection Regulations (GDPR). PLEASE NOTE: Should you need to obtain information from more than one organisation, a separate form will need to be completed, with the consent provided by the beneficiary, for each organisation you need to approach. 

The authorisation form now also explains that SSAFA has obtained consent to share the beneficiary's information with an external organisation to progress a live case/ application.
