This how to guide will work through what to do if a Beneficiary needs to be referred to the Regional Office. At any event or activity a volunteer may attend it is probable that potential beneficiaries will be present. It is important that all potential beneficiaries are referred to the RO and not allocated a caseworker by the Branch. This form can also be given to organisations who may have dealings with our beneficiaries and they can refer directly to the Regional Office.

The digital form can be completed online and sent to the RO via email.
Take a picture of the completed paper form and send that via email to the RO.
Initial Contact with potential beneficiaries
During any event or activity volunteers will come into contact with veterans/reserves/serving personal and their families who may require assistance from SSAFA.
Print off copies of the 3rd party referral form and take them to events with you. This will combat any WiFi issues at the event.
It is important to give the person the opportunity to make their own contact with the Regional Office if they prefer. There are regional contact cards available on the marketing hub to give out to potential beneficiaries. Go to: www.ssafa.online to access the marketing hub.
The Regional Office will contact the beneficiary in the first instance and signpost where necessary and assign a caseworker. This will depend on the needs of the beneficiary.
All Regional Offices have a group email address so there will always be someone to pick up the referral.
Always refer beneficiaries to the Regional Office.
1. Organisation name is the Branch name
2. Referrer's role is your volunteer role such as community engagement volunteer.
3. Telephone, there is no expectation to add a telephone number. Especially not your personal mobile number. This can be left blank. If anyone in the Regional Office needs to contact you they can email.
4. Email is your Assemble email address for ease of contacting and in line with GDPR. Do not use your personal email address here.
Do not provide any personal information for yourself on this form it is not necessary as the Regional Office can contact you on Assemble.
The next section is the details of the person being referred. See below picture. Make sure all the starred details are completed.

If completing the above on digital version you will be able to choose the service by clicking the arrow buttons in the box to show the drop down list.
The last section is a box for free text on the reason for the referral. Please write as much detail as is possible in this box. It is important the Regional Office have as much information as possible about the beneficiary.
